A Shit-A-Rein of This… and a Shit-A-Rein of That

And Why Great Practices Are Never Built by Accident

My wife’s grandmother was an extraordinary cook.

When someone asked her for a recipe, she would smile and say,
“A shit-a-rein, a shit-a-rein.”

A little of this. A little of that.

The result?

No one ever managed to duplicate her dishes.

A shame.

Years later, when colleagues began asking me what made my dental practice the envy of so many others, I realized I was giving the exact same answer.

A little marketing.
A little systems.
Good people.
Hard work.
Timing.
Luck.

My own professional version of “a shit-a-rein.”

And that, too, was a shame.

Because today, far too many dentists and healthcare professionals believe that truly exceptional practices are out of reach — reserved for the lucky, the naturally gifted, or the ones who somehow “figured it out” early.

Meanwhile…

Burnout is rising.
Margins are shrinking.
Teams are harder to find and harder to keep.
And too many talented clinicians feel trapped in businesses they once loved.

So, unlike Grandma Bertha, I decided to start writing the recipes down.

For more than forty years in practice — and right up until my last day of clinical dentistry on June 30th, 2020 — I became a student of my own success. I carefully cataloged the ingredients that actually mattered. I tested new ratios. I changed tools. I experimented with leadership styles, communication strategies, marketing approaches, financial systems, and team structures.

I studied with masters such as Dan Kennedy and Dr. Robert Cialdini.  Not dentists, but rather students and teachers of Persuasion and Influence.

Not to grow bigger.

But to grow better.

Better relationships with patients.
Better experiences for the team.
Better clinical outcomes.
Better balance for my family.
Better freedom to teach, write, give back, and enjoy life.

One of the most meaningful moments came when my son said to me,
“Dad, I don’t know if I’ll ever find something I love as much as you love dentistry.”

I was proud.

And quietly concerned.

Because passion doesn’t come from procedures. It doesn’t come from technology. And it certainly doesn’t come from insurance participation.

It comes from purpose.

From contribution.

From building something that works — for your patients, your team, and your life.

Over time, I realized something important:

There is a recipe for practice success.

And it can be taught.

That realization became the foundation for what we now call
